Discharge this week

Hospital discharge placement in New Jersey

If a discharge date just landed, start with a call.

Are you a hospital, rehab, or community care team? See how we work with professionals.

What a discharge week looks like

This service is for private-pay families. If the family is on or headed to Medicaid, MLTSS, or MLTC, we say so on the first call and hand them to the New Jersey ADRC at 1-877-222-3737.

A case manager says a parent cannot go home safely, and the family has a few days to find somewhere licensed, tour it, and get paperwork moving. It is the worst possible week to be reading directory listings.

Call (201) 686-8232 and tell us the discharge date. We shortlist licensed private-pay New Jersey communities that can take a new resident on that timeline, and we help you ask each one the questions that decide the answer.

What to gather before you call

  • The discharge date the hospital or rehab gave you.
  • The written list of help your parent needs now, from the therapy or nursing notes.
  • Current medications and who has been giving them.
  • Whether a two-person transfer or a wheelchair is involved.
  • Which towns work for the family members who will visit.
  • The monthly number the family can carry, as a range.

What actually slows a discharge move down

  • The community's own nurse assessment, which has to happen before they can accept.
  • Medical forms and a recent physician statement, including required test results.
  • A first month payment and a signed residency agreement.
  • Care level pricing that is only decided after the assessment.

Rehab is not the same as assisted living

Short term rehab is medical and time limited. Assisted living is housing with daily help. Memory care is assisted living built for dementia (see New Jersey memory care). A nursing home is a different license again, and it is not what we place families into.

If the clinical team says skilled nursing is needed, tell us and we will say so plainly rather than sell you a different setting.

Tell us the discharge date

Please do not type Social Security numbers, Medicare numbers, bank details, or medication lists.

What does your parent need help with? Check all that apply.
We never pass your details to a community or partner without asking you first and naming who receives them. We only contact you the way you choose. You don't pay. There's no fee.

How we handle your information

Frequently asked questions

Can you really move this fast?
Often yes, when the family is ready and a community has room. We do not promise a move-in date, because the community's assessment and paperwork decide the timeline.
Do families pay you?
You don't pay. There's no fee. A community may pay us a professional referral fee after a successful private-pay move in. We do not take a fee on Medicaid or Medicare funded placements.
Can you talk to the hospital case manager?
We are happy to work alongside them with your permission. They handle the medical discharge plan and we help the family compare housing options.
What if the family cannot afford private pay?
Say so early. We will point you to official New Jersey programs, and we take no fee on Medicaid or Medicare funded placements.
